2025-2026 Registration

1.) Complete Registration Form with $50 registration fee payable to: CES

Printed copies of forms are available in the school office. Registration is not complete without the $50 fee.

2.) Read Tuition Letter

3.) The following items will be collected after registration is confirmed:

- Submit online Student Data form using this link → Student Data Form

- Current Physical (child health assessment) signed/dated by a Physician (dated Sep. 1, 2024 - Sep. 1, 2025) → Health Forms

- Current Immunization Record (provide paper copy or email to ashleigh.harp@usd267.com)

- Submit Tuition Payment Form after screenings are completed in August → Authorization of Payment (attach a voided check for option 2)
